Support for the Homeless

PATH Program

New Horizons is funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) to provide Projects for Assistance in Transition from Homelessness (PATH). PATH Workers and homeless consumers work together to:

  • Secure basic shelter in a homeless shelter, camp, or permanent housing
  • Access needed psychiatric services and medications
  • Apply for income and support programs such as Social Security, Medicaid, and food stamps
  • Make a plan to reverse or stabilize the factors that led to homelessness
  • Secure a stable future
